Respirator Fit Test

All booking appointments are per person and will be conducted at our office located at 67 Atherton Road, Oakleigh, VIC 3166.

Please note this booking form will not take online payments. Payment will be requested at the time of the appointment. No cash payments accepted, card ONLY.

Should you require a booking outside our online availability, please contact us on (03) 9563 2234 to make arrangements.

If you require a half/full face respirator fit test (resusable masks), please ensure to specify the make and model of the mask in the additional notes section.

Please also advise any accessibility requirements.

! IMPORTANT !

• Please arrive 10 minutes prior to your appointment time.
• Fit tests will only be carried out on clients who are clean shaven or have no hair between their face and the fitting surfaces of the respiratory face piece.
• No eating, drinking or vaping/smoking 30 minutes prior to the booking time.
